School Forms

Parents/Guardians requiring proof of enrollment (i.e., CRA, CIC, Citizenship, etc.) for their student(s), please email the office at jdes@sd35.bc.ca,  with the following information, students name, grade, division, parents name and email.  There is a cost of $20 and may take 3-5 days to be ready for pick up at the office. 



STUDENT MISSING EXTENDED SCHOOL TIME – Please complete the Student Missing School Time Form, if your student(s) are going to be away from school due to an extended vacation, or a family leave


Moving/Withdrawing – PLEASE LET US KNOW IF YOU ARE YOU MOVING

Parents are asked to inform the school as soon as possible if/when they know that their children will be leaving our school or not returning in the fall. Please complete our Withdraw form and email it jdes@sd35.bc.ca



Volunteers and volunteer drivers

Volunteers – If you wish to volunteer in a classroom or for field trips during the school year, parents and guardians are required to complete the Volunteer Safety Check Application and get your Police Information Check (PIC, formerly called a Criminal Record Check/CRC) every school year.

Field Trip/Event Volunteer Drivers –  bring student(s) booster seat if required.

To protect our children and you as a driver, we ask that you complete the following. You will need to provide the school a copy of your driver’s license, driver’s abstract and your current Autoplan Insurance Policy, the policy must include a minimum $2 million liability insurance.

If you are able to drive please bring copies of the following to your students teacher. The office does not have time to copy documents for families

  • Driver / Volunteer Forms
  • Copy of driver’s license Class 5 (NO “N” or “L” )
  • Copy of drivers abstract will need “Personal driving record” https://onlinebusiness.icbc.com/clio/
  • Copy of the first two pages of their car insurance and they must have $2,000,000 liability. One page has vehicle information, second page will have insurance coverage.

All forms will be shredded at the end of the school year, as people’s insurance and licenses may expire



Medical Concerns

Please make sure that you have indicated any medical concerns or information that the school should be aware of (e.g., asthma, allergies, etc.  please complete medical forms if needed)


Aboriginal Ancestry

Self-Identification of Aboriginal Ancestry Form

**Please fill out only if student has Aboriginal ancestry – one form per child**

Aboriginal Ancestry is determined on a voluntary basis through self-identification. This includes First Nations (Status or Non-Status), Metis or Inuit Ancestry. No documentation other than this self-identification is required and the ancestry can go back several generations.
